SUMMARY OF POSITION

The MDS Coordinator position coordinates the MDS process for all residents by performing all Medicare and Non-Medicare admissions and assessments; assists with facilitation of Medicare and Non-Medicare discharge planning; facilitates meetings with residents and/or family to discuss/advise on Medical requirements and eligibility; provides Medicare training to nursing as needed; prepares all Medicare requirements and PPS documents in accordance with federal mandated Medicare rules and regulations for admission and discharge eligibility.

GENERAL REQUIREMENTS

  • Valid and active state-issued Registered Nurse (RN) license.
  • Two years of experience in long-term care preferred.
  • Three or more years of MDS 3.0 experience - scheduling and completing both OBRA and PPS MDS’s.
  • Must possess sound knowledge of nursing principles, procedures and standards of practice as determined by state and federal regulations.
  • Ability to read, analyze, interpret, effectively present information.
